Connected Healthcare Solution, LLC is proud to partner with Iroquois Memorial Hospital (IMH) who is seeking a compassionate and collaborative Primary Care Physician to join our Rural Health Clinic team. As a Critical Access Hospital serving East Central Illinois and Northwest Indiana, IMH offers physicians the opportunity to practice broad-spectrum primary care while working closely with specialists, hospital providers, nursing homes, and community partners.

Position

Highlights
  • Outpatient primary care practice in a Rural Health Clinic setting
  • Opportunity to build long-term relationships with patients and families
  • Collaborate closely with experienced Nurse Practitioners, specialists, hospital providers, and care management teams
  • Participate in population health initiatives, including Annual Wellness Visits, chronic disease management, and preventive care programs
  • Opportunity for nursing home rounding.
  • Access to on-site laboratory, imaging, specialty clinics, rehabilitation, and hospital services

What We're Looking For
  • MD or DO licensed (or eligible for licensure) in Illinois
  • Board Certified or Board Eligible in Family Medicine or Internal Medicine
  • Commitment to providing high-quality, patient-centered care
  • Strong communication and team collaboration skills
  • Interest in rural healthcare and serving underserved communities
  • Willingness to participate in quality improvement and population health initiatives
