Position: Physician (MD/DO) Skilled Nursing Facilities Part Time
Location: Flanagan

About the Role

We're looking for an experienced, autonomous Physician
to join our clinical team part-time, providing primary and episodic care to residents in skilled nursing facilities. This is an independent role built around continuity you'll manage your own panel and build long-term relationships with patients and their families.

Your Day-to-Day

You'll see roughly 16–25 patients a day a mix of long-term chronic care and acute issues. That means annual wellness visits and H&Ps,managing chronic and co-morbid conditions, responding to acute changes,reviewing meds and orders, and coordinating with collaborating specialists and facility staff to keep care seamless. You'll also stay in touch with families and join care conferences where it makes sense.

Qualifications
  • MD or DO from an accredited medical school.
  • Board Certified (BC) required
    Internal Medicine, Family Medicine, or Geriatrics (or equivalent).
  • Valid, unrestricted physician license in the state of operation.
  • Successful completion ofan accredited residency program.
  • Skilled nursing orlong-term care experience strongly preferred.
  • Strong independent clinical judgment and the ability to move quickly in a fast-paced setting.
