• MD or DO – Required
• Completion of an accredited Family Medicine residency program in the United States
  - Required

• Active DEA license or ability to apply for such license prior to or upon hire
  - Required
• Active Illinois medical license
  - Required
• Board Certification or Board Eligibility in Family Medicine. Board certification must be obtained within 5 years
  - Required
