General Summary

Staff will provide and coordinate cardiopulmonary rehabilitation activities for Phase II and Phase III patients to assure compassionate patient care is delivered. Provides direct patient care in compliance with professional practice and clinic standards. Staff will promote a collaborative, interdisciplinary approach towards patients, families and all members of the care delivery team.

Principle Duties and Responsibilities
  • Follows guidelines for patient assessments. Individualized treatment plans (ITPs).
  • Formulates individualized predefined goals in a consistent and continuous manner. Follows utilization and adherence to pre-determined standards, which define the structure, process, and desired patient outcomes.
  • Identifies patient learning needs and readiness for learning. Implements patient/significant-other instruction to meet self‑care needs as defined in standards of care and patient teaching protocols.
  • Performs documentation responsibilities following all existing structure standards and guidelines for forms used in the Division of Nursing and Cardiopulmonary Rehab.
  • Functions with an awareness of and adherence to safety requirements as identified in existing standards.
  • Performs efficiently in emergency patient situations following established protocols, remaining calm, informing appropriate persons, and documenting events.
  • Demonstrates applied knowledge base in areas of patient care resources/references, protocols, pertinent drugs, standards of care, billing practices and significant lab values.
  • Participates actively in staff development activities by sharing knowledge with others and using appropriate teaching/learning strategies to assist peers.
  • Demonstrates self‑directed learning and participation in continuing education to meet own professional development.
  • Seeks validation of knowledge base, skill level, and decision making as necessary and assertively requests guidance in areas of question.
  • Participates in development of standards policy and protocols at the unit‑specific level, demonstrates an attitude of acceptance/importance of standards in all professional practice. Strives to achieve minimum compliance levels and uses “professional modification” measures with sound clinical judgment.
  • Demonstrates willingness and ability to effectively coordinate patient care/unit activities among nursing staff, physicians, and support services in the care manager role.
  • Demonstrates effective organization and priority setting abilities in using self, human, and material resources in a time‑oriented and cost‑effective manner.
  • Demonstrates effective verbal and written communication skills by always attempting to receive and give information in a timely, concise, clear and positive manner.
  • Formulates and uses effective working relationships with all health‑care team members, patients, and families.
  • Performs other duties as assigned.

  • Floor to waist – 40 pounds
  • 14" to waist – 50 pounds
  • Waist to shoulder – 20 pounds
  • Shoulder to overhead – 10 pounds
  • Carry 40 pounds for 30 feet
  • Push 40 pounds for 30 feet
  • Pull 40 pounds for 30 feet
  • Emotional stability to deal with high‑prolonged stress of staff teamwork, patient care, and physical environment.
  • Positively interacts with medical staff and support services during times of stress and constant change.
  • Keen mental faculties to perform assessment and decision‑making skills related to patient care and departmental duties.
  • Strong intellectual abilities of critical thinking, comparative analysis, statistical control application, and decision‑making.
  • Physical ability to perform CPR and occasional lifting whenever needed.
  • Twisting, turning and stretching maneuvers to participate in patient care.
