• Evaluate patients and create individualized, goal-driven treatment plans
• Deliver skilled physical therapy interventions to promote safe mobility
• Document clinical progress clearly and timely in the medical record
• Collaborate with nurses, OT, SLP and case managers to optimize patient outcomes
• Educate patients and families on home programs and safe discharge plans
• Contribute to a positive, team-focused culture that prioritizes patient-centered care

• Licensed Physical Therapist (PT) or Doctor of Physical Therapy (DPT) — DPT/ PT credential indicated in the job skills
• New grads welcome — Years of experience: any, including New Grad
• Comfort working in a Skilled Nursing Facility (SNF) practice setting
• Strong communication skills and ability to work collaboratively with interdisciplinary teams
• Current CPR/BLS recommended (confirm facility requirement upon interview)

• DPT or PT degree (Physical Therapy). Job indicates PT / DPT credentials.

• Skilled in patient assessment, mobility training, and therapeutic exercise
• Knowledge of documentation best practices and care plan development
• Able to perform safe transfers and fall-risk mitigation strategies
• Good clinical reasoning, time management, and patient education skills
• Comfortable working with geriatric and post-acute populations
