Job Description

Identifies and resolves issues affecting the delivery of patient care with the team. Performs quality assurance studies and assists with implementing quality assurance and improvement initiatives. Guides interdisciplinary team members in performance improvement and incorporates evidence-based practice protocols into daily operations of the stroke program. Ensures a high level of data accuracy and quality for stroke program reporting. Collaborates with community and professional organizations to further education and awareness about stroke prevention and treatment.

Provides outstanding customer service and maintains respectful relationships. Other duties as assigned. Monitors departmental compliance with regulatory and compliance requirements. Assists with the marketing and advertising of programs and services. Acts as a key decision maker for interdisciplinary teams responsible for monitoring and improving program performance. Facilitates the development and implementation of stroke-related educational initiatives for staff, patients, community, and emergency medical services.

UChicago Medicine Advent Health Hinsdale Stroke Program Coordinator Exempt will be responsible for coordinating and advancing clinical excellence in stroke care across the facility. Collaborates with medical staff, nursing leadership, and interdisciplinary teams to ensure timely, evidence-based, and high-quality stroke care throughout the continuum of care. Develops, implements, and monitors stroke care protocols, pathways, and quality initiatives in alignment with regulatory and accreditation standards.

Evaluates stroke care outcomes, identifies opportunities for improvement, and supports performance improvement efforts to enhance patient safety and clinical results. Serves as a resource for staff education by developing and delivering training on stroke care best practices, updates in standards of care, and program expectations. Promotes consistent care delivery, interdisciplinary communication, and continuous program development to support optimal stroke patient outcomes.
