Physical Therapist​/Physical Therapy Assistant, PRN

East Bank Center, the Rockford area's only short-term only, nursing and rehabilitation center is in need of an Physical Therapist or Physical Therapy Assistant. If you pride yourself on delivering the best therapy services to patients who are recovering from a medical or surgical event then we want to invite you to apply for this position.
MUST have physical therapy or physical therapy assistant degree to apply.

Responsibilities:

Evaluate patient's functional status, muscle function, endurance, visual coordination, written and verbal communication skills, self care ability, work capacity, etc. Develop treatment program and establish goals for improved function. Provide therapy services established by the treatment program and goals for improved function. Maintain appropriate clinical records and progress notes.

Requirements

• Graduate of an accredited school for Physical Therapy.
• Hold a valid state license.
