Travel Skilled Nursing Facility (SNF) Occupational Therapist – Prophetstown, IL

Jackson Therapy Partners is looking for a travel Occupational Therapist to provide services in a skilled nursing facility for a 13‑week assignment.

• Facility Type:
  Skilled Nursing Facility
• Shift: 8‑hour days, 5 days per week
• Duration: 13 weeks

• Provide occupational therapy services five days per week to patients with injuries, illnesses, or disabilities.
• Document patient progress using electronic medical records.
• Collaborate with interdisciplinary team members to develop and implement care plans.

• Bachelor's, Master’s, or Doctoral Degree in Occupational Therapy from an accredited program.
• Active Illinois Occupational Therapy license.
• Experience working in a skilled nursing facility.
• BLS Certification from AHA or ARC (may be required).
