Location: Fairfield
NATURE OF WORK Fairfield Memorial Hospital is seeking a highly skilled, patient focused Physical Therapist to support the daily operations of the FMH Physical Therapy Department. This role is essential in delivering high quality rehabilitative care by evaluating patients, developing individualized treatment plans, and administering therapeutic interventions that restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities.
The Physical Therapist works directly with patients across all age groups and plays a key role in ensuring a positive, professional, and supportive therapy experience. In addition to direct patient care, the Physical Therapist collaborates closely with physicians, Physical Therapist Assistants, and interdisciplinary clinical staff to maintain efficient workflows, uphold safety and infection control standards, and support FMH’s mission of providing safe, reliable, and compassionate care.
KEY RESPONSIBILITIES Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
Perform assessments and reassessments across all age groups, ensuring safe and effective therapy delivery.
Monitor treatment effectiveness and recommend modifications to the referring provider as appropriate.
Address patient pain that interferes with functional progress and coordinate with physicians for intervention.
Provide patient and family education, ensuring understanding of therapy goals and home programs.
Demonstrate proficiency in physical therapy modalities, therapeutic techniques, and safe equipment operation.
Maintain accurate, timely documentation in accordance with departmental and hospital standards.
Collaborate professionally with physicians, staff, patients, and families to support coordinated care.
Uphold patient dignity, cultural sensitivity, and confidentiality in all interactions.
Participate in departmental operations, safety initiatives, infection control, and equipment maintenance.
Assist with orientation, training, and support of Physical Therapist Assistants and other staff.
Promote positive customer relations and uphold FMH’s Standards for Service Excellence.
COMPENSATION: $74,880-$118,560 annually

EDUCATION/QUALIFICATIONS
Required:
Current Illinois State Licensure as a Physical Therapist;
Graduate of an accredited Physical Therapy program.
Preferred:
Prior experience in a hospital or outpatient therapy setting;
Experience working with diverse patient populations across the lifespan.
EXPERIENCE Preferred:
Clinical experience in physical therapy through internships or prior employment;
Experience with therapeutic equipment, physical therapy modalities, and patient education.;
Ability to work independently across multiple settings.
New graduates welcome.
SKILLS Strong clinical assessment and treatment planning abilities
Excellent communication and interpersonal skills
Ability to read and interpret safety rules, operating instructions, and procedure manuals
Ability to write routine reports and communicate effectively with groups
Basic computer proficiency
Knowledge of anatomy, physiology, physical medicine, and therapy equipment operation
Strong communication skills
Efficient in time management
