Position Summary

The Community Health Worker / Welcome Center Caseworker assists all individuals including immigrant, refugee, and limited English proficiency individuals and families with accessing services and understanding available resources. This role serves as a trusted, frontline support for community members, helping to reduce barriers to health care, public benefits, and essential services through culturally responsive navigation, education, and advocacy.

The Community Health Worker / Welcome Center Caseworker acts as a liaison between the community and health, social service, and public systems to facilitate access to care, improve service quality, and promote culturally competent service delivery. Services and resources supported include, but are not limited to, public benefits, healthcare, housing, employment, education, legal aid, DMV services, naturalization and immigration support, documentation, food security, and other basic needs.

In addition to direct client services, this position plans, coordinates, and participates in outreach programs and community activities to provide education, assistance, and referrals to new and existing individuals and families. The role also builds individual and community capacity by increasing health knowledge, self-sufficiency, and system navigation skills through outreach, community education, informal counseling, social support, and advocacy.

The Community Health Worker / Welcome Center Caseworker regularly engages in community outreach and educational activities, which may require a flexible schedule, including occasional evenings and weekends.

Salary Range

$18.00 - $21.00 per hour based on experience

Responsibilities
  • Assist individuals and families with accessing health, social, and public services and understanding available resources.
  • Conduct one-on-one client intakes, screenings, and needs assessments to provide education, assistance, referrals, and follow-up.
  • Provide case management and care coordination for clients requiring more intensive support.
  • Screen for Social Determinants of Health (SDOH) needs and connect clients to appropriate services.
  • Become a trained Insurance Navigator to assist clients with Medicaid and Get Covered Illinois insurance enrollment and renewals.
  • Educate individuals and families on health insurance options, public benefits, and system navigation.
  • Provide culturally and linguistically appropriate health education, disease prevention, and chronic disease management support.
  • Build trusting, empathetic relationships with clients and provide coaching, motivation, and social support.
  • Conduct outreach activities, present at community events, and deliver educational workshops on relevant topics.
  • Support grant-funded programs by completing required activities, documentation, outreach, and reporting to meet grant deliverables.
  • Collaborate with the Welcome Center Coordinator, Outreach Coordinator, and colleagues to plan, develop, and deliver curricula and educational programs and workshops.
  • Collaborate with internal teams and community partners to coordinate services and maintain referral networks.
  • Advocate for client and community needs related to healthcare access, housing, food security, and other basic needs.
  • Accurately document services provided, track data, and complete required reports for program and grant compliance.
  • Provide CHW services and complete required documentation to support Illinois Medicaid billing and reimbursement, as applicable.
  • Participate in ongoing training to remain current on public benefits, insurance programs, immigration-related policies, and CHW best practices.
  • Perform all duties in a manner that promotes equity, cultural responsiveness, and respect.
