Overview

Location:

Hinsdale Hospice Office – 119 E Ogden Ave, Hinsdale, IL 60521

Hours:

Monday – Friday 8:00 AM – 4:30 PM, every other weekend. This position supports patient referrals and admissions within the hospice care context.

Responsibilities
  • Provide telephonic support to patient referral sources to initiate patient admission.
  • Document patient demographic information, condition, referral source, and other pertinent data in electronic medical records.
  • Dispatch nurses (e.g., Admissions Nurses or Hospital Liaison Nurses) to assess patients and begin the admission process.
  • Update and check nurse schedules to ensure appropriate appointments are scheduled.
  • Communicate information to and from nurses regarding patient status and update patient records accordingly.
  • Coordinate with nursing staff to facilitate patient admissions and ensure timely service delivery.
  • Maintain effective communication with patients, families, and healthcare providers throughout the admission process.
  • Order special equipment or supplies as requested by the nursing staff.
  • Track and update pending patient admissions by contacting patients or referral sources to schedule nurse visits.
  • Participate in staff meetings, department meetings, in-services, and related activities.
  • Attend educational and developmental seminars and meetings as deemed appropriate.
  • Create daily reports and communicate new patient admissions and referral outcomes to relevant individuals.
  • Obtain consents or provide information to patients' family members as needed.
  • Perform other duties as assigned.
Knowledge, Skills, and Abilities
  • Ability to read, write, and type legibly; effective spoken English to carry out job requirements.
  • Knowledge of electronic medical records.
  • Ability to complete hospice orientation and training as necessary.
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and internet software.
  • Knowledge of medical terminology.
  • Ability to think critically regarding the admission process.
Education and Experience
  • Education:

    High School graduate or equivalent (Required);
    Associate degree preferred.
  • Field of Study:
    Non-applicable.
  • Work Experience:

    3+ years of administrative or clerical experience (Required).
  • Healthcare experience, call center experience or related (Preferred).
