Gibson Area Hospital & Health Services in Gibson City, IL seeks a Social Worker to provide case management and psychosocial support, helping patients and families meet identified needs and plan post-discharge services.
The position works under the LCSW Field Instructor, coordinating with the multidisciplinary team to ensure safe transitions of care, effective communication, and timely discharge planning in a busy healthcare setting. Monday–Friday 8 am–4:30 pm with occasional weekend coverage.
