Job Title
Provides therapeutic interventions aimed at fostering personal growth and resilience in clients with behavioral health diagnoses.
Essential Duties & Responsibilities- Engages with newly assigned clients and their families by documenting contact within 2 business days of the referral.
- Conducts intake for clients entering services, ensuring understanding of all forms and obtaining necessary signatures.
- Develops individualized care plans that are person centered, family centered, and service centered.
- Assists clients in linking with internal and external resources as outlined in service plans.
- Maintains documentation of services provided and submits for review as required.
- Monitors client progress in treatment and participates in service plan reviews.
- Provides community support, care coordination/case management, and crisis intervention services.
- Knowledge of community geographical awareness and of community and agency resources.
- Skill in interviewing, assessment, and counseling.
- Proficiency in customer service techniques, demonstrated through effective resolution of client issues.
- Advanced problem-solving skills, evidenced by successful project outcomes.
- Familiarity with digital communication systems, including Electronic Health Record for managing client interactions.
- Ability to conduct thorough assessments and form diagnoses, utilizing standardized tools and methodologies.
- Expertise in virtual communication platforms, ensuring clear and effective remote interactions.
- Strong analytical skills, applied in evaluating individual needs and resources.
Education Level
Master's Degree in Human Services field, licensure eligibility required
