Therapist – Physical Therapist

Job Summary:

The Physical Therapist provides specialized rehabilitation services to patients in a Skilled Nursing Facility setting. They assess patient needs, develop treatment plans, and implement therapies to improve mobility and functional independence.

Responsibilities:

• Conduct comprehensive evaluations of patient's physical capabilities and limitations.
• Develop and implement individualized treatment plans based on assessment findings.
• Administer therapeutic exercises and manual therapies to promote recovery.
• Monitor patient progress and adjust treatments accordingly.
• Educate patients and their families on therapy exercises and home programs.
• Document all care activities accurately in patient records.

Qualifications:

• Hold a valid state license or certification as a Physical Therapist.
• Demonstrate a minimum of 1-2 years of outpatient or SNF therapy experience.
• Possess strong interpersonal and communication skills.
• Be proficient in documenting patient care and treatment plans.
• Maintain current CPR and first aid certifications.
• Ability to work effectively within a multidisciplinary team.
