• Providing direct patient care to residents, including assistance with daily living activities, vital signs, and wound care
• Maintaining accurate and timely documentation of patient care and progress
• Collaborating with the healthcare team to develop and implement individualized care plans
• Promoting a culture of respect, dignity, and compassion in our care environment

• Assist residents with activities of daily living, including bathing, dressing, and grooming
• Monitor and report changes in resident condition to the healthcare team
