In-Home Traveler Physician

Signify Health seeks a Full-Time In-Home Traveler Physician to perform comprehensive clinical evaluations for adults in their homes across assigned regions. You'll assess medical, functional, and social needs; reconcile medications; order appropriate tests; and document findings in our technology platform to support value-based care. Collaborate virtually with care teams, use data insights to close care gaps, and help connect vulnerable patients to needed services.

Responsibilities

• Perform comprehensive in-home medical evaluations for adults, focusing on complex and vulnerable populations.
• Conduct detailed histories, physical exams, and chronic disease assessments.
• Reconcile medications, review diagnostics, and identify gaps in care and safety risks.
• Document encounters thoroughly and accurately within Signify Health's technology platform and EHR systems.
• Collaborate with remote interdisciplinary teams to support care plans and value-based care goals.
• Educate patients and caregivers on conditions, medications, and preventive care.
• Order or recommend appropriate labs, imaging, and follow-up with primary or specialty care.
• Adhere to clinical, regulatory, and HIPAA requirements while working independently in the field.
• Travel regularly within assigned regions to complete scheduled in-home visits.
• Leverage data and analytics to inform clinical decisions and improve patient outcomes.
