Ardor Health Solutions is seeking an Occupational Therapist for a travel SNF assignment in Mount Vernon, Illinois
.

This is a great opportunity for an OT who enjoys working with an adult and geriatric rehabilitation population
, helping patients regain independence following illness, injury, surgery or hospitalization.

• Skilled Nursing / Rehab setting
• Adult and geriatric population
• Short-term rehabilitation and longer-term skilled needs
• Individualized therapy plans
• Interdisciplinary rehabilitation environment
• Focus on maximizing independence and safely transitioning patients to their next level of care

• Complete OT evaluations and reassessments
• Develop individualized plans of care
• ADL and self-care retraining
• Upper-extremity strengthening, ROM and coordination
• Functional mobility and transfer training
• Adaptive equipment and compensatory strategies
• Cognitive and safety interventions as appropriate
• Patient and caregiver education
• Participate in discharge planning
• Collaborate with PT, SLP, nursing and the interdisciplinary care team

• Graduate of an accredited Occupational Therapy program
• Illinois OT license or ability to obtain prior to start
• Current BLS
• SNF or geriatric rehab experience preferred
• Strong documentation and time-management skills
