Job OverviewThe LPN Medication Care Manager provides quality care by administering medications and treatments safely, in accordance with laws, regulations, and Sunrise policies, to promote the highest quality of life for residents.
Responsibilities & Qualifications Essential Duties Medication Administration- Administer medications and treatments safely and organized.
- Document medication administration accurately in the electronic health record.
- Follow the Six Rights of Medication Pass.
- Ensure timely medication passing using a mobile cart.
- Maintain confidentiality of resident information.
- Report concerns and medication issues to the RCD or Wellness Nurse.
- Restock medication supplies and assist with medication checks.
- Assess residents for "as needed" medications and document appropriately.
- Count narcotics with a colleague each shift and report discrepancies.
- Keep medication areas clean and organized.
- Address resident and family concerns promptly.
- Participate in daily meetings and develop individualized care plans.
- Transcribe orders accurately.
- Ensure compliance with safety and occupational health regulations.
- Practice safety procedures, including PPE and hazard reporting.
- Report incidents and unsafe conditions immediately.
- Handle hazardous materials and exposure risks properly.
- Work effectively as part of a team.
- Participate in training and meetings.
- May serve as shift supervisor or supervise other medication care managers.
- Maintain required training and certifications.
- Decision-making and judgment in residents' best interests.
- Calmness in difficult situations.
- Handling multiple priorities.
- Effective communication skills.
- Problem-solving abilities.
- Licensed practical nurse (LPN, LVN) or equivalent license.
- Current certifications in CPR and First Aid.
- At least 18 years old.
- Experience with seniors preferred.
- Desire to serve seniors.
- If driving, must understand safety duties related to operating a vehicle.
