Hospice Nurse Practitioner PositionPurpose:
The Hospice Nurse Practitioner serves as a primary level care provider under the clinical supervision of the Hospice Medical Director. The NP (Nurse Practitioner) will make face-to-face assessments of hospice patients in a variety of settings under all levels of hospice care. The NP will collaborate with the members of the IDT (Interdisciplinary Team) to assess client eligibility for hospice and provide interventions as required for pain and symptom management.
The NP demonstrates expertise in patient care. The NP may serve as a liaison with other hospice referral sources, providing consultation when necessary. The NP will maintain high standards of integrity and business ethics and provide all patient care in accordance with current professional standards of practice and regulatory requirements.
- Makes visits to clients in their place of residence and/or in hospital setting
- Consistently and accurately assesses clients at admission, at required time points, and as needed, utilizing appropriate standardized documentation. Assesses the physical and psychosocial status of clients through interview, health history review, physical examination, and, when appropriate, diagnostic studies.
- Evaluates clients for ongoing eligibility for hospice services by reviewing IDT (Interdisciplinary Team) plan of care, documentation and diagnostic data, and through collaboration with the Medical Director and client’s Attending Physician.
- Support On-Call MD/NP duties.
- Graduate of an approved Nursing education program
- Master’s Degree in nursing
- Certified/licensed as a Nurse Practitioner in state of Ohio
- DEA License in Ohio if required
- Must have full (non-restricted) primary active medical staff membership at local Trinity Hospital, and remain in good standing with appropriate clinical privileges
- In good standing/able to participate in the Medicare & Medicaid programs
- Minimum of 5 years nursing experience; clinical nursing care in oncology, hospice, geriatrics, intensive care preferred
- Exhibits strong communication and interpersonal skills, with a special emphasis on grief/loss management.
- Must have current Driver’s license and reliable transportation to and from work site.
- Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics, and adhere to the Compliance Program.
