Location
Position Summary:Provides excellent, professional day-to-day hands‑on personal care to individuals with chronic, serious, or life‑limiting conditions. CNAs must exhibit competencies required by law and abide by Four Seasons policies in providing personal care to patients, as directed by the written plan of care.
- Practice within the scope of license, certification or registration, where applicable, and as required by law and regulations as delegated to and directed by the registered nurse
- Participates in the coordination of the patient’s hospice care in accordance with professional standards of practice
- Collects data for ongoing interdisciplinary comprehensive assessments
- Participates in the development and evaluation of the plan of care by reporting observations
- Contributes to patient and family counseling and education
- Perform duties in compliance with Medicare Condition of Participation
- Practice safety and infection control methods
- Provision of hands‑on personal care
- Works to achieve performance goals, pursue progress toward individual development plan (IDP) and demonstrate Four Seasons values and competencies
- Must be willing and able to care for patients with infectious disease processes unless approved accommodations
- Knowledge of proper body mechanics
- Excellent interpersonal skills
- Excellent written and oral communication skills
- Ability to work under pressure to meet deadlines
- Ability to take initiative and utilize innovative techniques and ingenuity
- Ability to interact with various departments throughout the organization
- Ability to work flexible hours, if needed

Education:
- High School Diploma/GED
Experience:
- No more than a 24 month lapse in providing clinical care services upon the completion of a State‑Accredited Certified Nursing Assistant course
- 0 – 1 year of clinical experience.
Licenses &
Certifications:

- Valid NC CNA certificate
- Valid driver’s license and proof of automobile insurance
Physical Requirements:
- Walking, standing, sitting, bending, reaching, twisting, crouching/squatting, kneeling
- Flexion/Extension of arms/wrist
- Hand dexterity to include gripping, pushing/pulling and writing
- Lifting/carrying of 100 lbs. often
