We are currently seeking an APP (FNP, NP, or Adult Gero NP) or PA based in Flanagan, IL to serve local skilled nursing facilities within a mission‑driven care community.

• Provide high‑quality, patient‑centered care to residents in skilled nursing facilities.
• Conduct daily rounding, prescribe medications and treatment plans, and address acute/emergent issues.
• Communicate with facility nursing staff via phone or secure messaging as needed.
• Maintain reliable on‑site attendance for scheduled shifts.
• Ensure compliance with clinical standards, facility policies, and regulatory requirements.

• 6‑hour facility shifts for full‑time positions (rounds generally start between 7‑10 a.m.).

• Voice dictation and AI Scribe for fast and accurate documentation.

Advanced Practice Providers must hold an active U.S. license and be board certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants). Qualified specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
