Licensed Practical Nurse

*** 12 Hour Shifts (6p-6a)

The LPN is responsible for providing direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by nursing assistants. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing to ensure that the highest degree of quality care is maintained at all times.

• Direct the day-to-day functions of the nursing assistants. Meet with assigned nursing staff and support personnel to plan shift services, programs, and activities. Make written and oral reports/recommendations concerning shift activities as required. Admit, transfer, and discharge residents as required. Provide leadership to nursing personnel assigned to your unit/shift.
• Complete and file required recordkeeping forms and charts upon resident admission, transfer, or discharge. Receive and transcribe telephone orders from physicians and record on the physician's order form. Chart nurse’s notes in an informative and descriptive manner reflecting the care provided.
• Prepare and administer medications as ordered by the physician.

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below are representative of the knowledge, skill, and/or ability required.

• Licensed Practical Nurse with current unencumbered state licensure.
• Current CPR Certification is required.
• Experience in Long Term Care is preferred.
• Must be able to work 12 hours a shift.
