Care Diem Home Health is seeking compassionate and reliable RNs to provide skilled home health nursing services to patients in LOVES PARK
. Our mission is to help clients remain safely at home while receiving top-quality, personalized care.
- Deliver skilled nursing care under a physician’s order
- Complete assessments, medication administration, and wound care
- Monitor and document patient progress using our electronic charting system (Axxess)
- Educate patients and families on health conditions and care
- Communicate with physicians and the care team
- Travel to patient homes
- Current RN license in the state of Illinois
- 1+ year nursing experience (home health preferred)
- CPR certification (BLS)
- Reliable transportation, valid driver’s license, and auto insurance
- Strong clinical judgment, communication, and independence
- Flexible scheduling (FT, PT, PRN, Per Visit Based)
