• Provide direct patient care to residents, including administering medications, performing vital signs, and monitoring patient conditions
• Assist with daily living activities, such as bathing, dressing, and feeding, as needed
• Collaborate with the healthcare team to develop and implement individualized care plans
• Maintain accurate and up‑to‑date records of patient care and treatment
• Participate in ongoing education and training to stay current with industry developments and best practices
• Utilize strong communication skills to effectively document patient information, communicate with interdisciplinary team members, and educate patients and families on care instructions

• Work in a fast‑paced and dynamic environment that values teamwork and collaboration
• Communicate effectively with patients, families, and healthcare professionals to provide comprehensive care
• Work closely with the interdisciplinary team, including physicians, social workers, and therapists, to develop and implement individualized care plans

• Current LPN license in the state of Illinois
• High school diploma or equivalent required; associate’s degree or higher preferred
• Minimum 0 years of experience in a healthcare setting; experience in a long‑term care or rehabilitation setting preferred
• Strong communication and interpersonal skills
• Ability to work collaboratively as part of a team
• Strong organizational and time management skills
