Graham Healthcare Group is seeking a Registered Nurse (RN) Hospice for a nursing job in West Dundee, Illinois.

What You’ll Do
  • Deliver compassionate, patient‑centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life.
  • Perform comprehensive patient assessments, symptom and pain management, and individualized end‑of‑life care planning.
  • Educate and support patients and families through the end‑of‑life journey, ensuring they feel informed, prepared, and deeply cared for.
  • Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
  • Work independently, but never alone.
  • Travel to patient homes within the assigned territory.

RN Qualifications
  • Active Registered Nurse license in the state you work.
  • Two years of RN experience with the clinical expertise to deliver quality patient care; hospice experience preferred.
  • Current driver’s license and ability to spend approximately 20% of the day driving to/from patient locations.

Compensation information:
The low end of the range includes salary and first‑year success bonus. The high end of the range is inclusive of productivity bonuses and additional shift work that is paid at a premium (e.g., after hours, weekends, on‑call). Compensation potential varies by market.
