8:00 AM – 4:30 PM

Participation in the on‑call rotation is required.

Compensation
  • Registered Nurse (RN): $33.00–$35.00 per hour (Based on Experience)
  • Licensed Practical Nurse (LPN): $29.00–$31.00 per hour (Based on Experience)
  • $10,000 Sign‑On Bonus paid over one year.

Position Responsibilities
  • Coordinate and oversee the MDS assessment process.
  • Complete and submit accurate, timely MDS assessments in accordance with CMS guidelines.
  • Lead interdisciplinary care plan meetings and collaborate with department leaders.
  • Monitor case mix and reimbursement opportunities.
  • Maintain compliance with federal and state regulations.
  • Support quality measures, survey readiness, and regulatory compliance initiatives.
  • Work closely with nursing leadership to promote outstanding resident outcomes and documentation accuracy.
Qualifications
  • Current Illinois Registered Nurse (RN) or Licensed Practical Nurse (LPN) license required.
  • Previous MDS and care planning experience required.
  • Strong knowledge of Medicare, Medicaid, and reimbursement systems preferred.
  • Excellent communication, organizational, and leadership skills.
  • Strong attention to detail with the ability to prioritize multiple responsibilities.
  • Long‑term care experience preferred.
