Location: Staunton
Matrix Medical Network is seeking licensed Nurse Practitioners or Physician Assistants to perform in-home Comprehensive Health Assessments (CHA) across the United States, including Illinois. Visits last up to one hour and focus on health history, medications, social factors, and potential care gaps to improve overall outcomes.
You will work with a multidisciplinary team, administer tests as needed, and document findings in the patient record. Travel to member homes or SNFs is required.
