The MDS Coordinator RN is responsible for coordinating, scheduling, and completing accurate MDS assessments while maintaining compliance with federal and state regulations. This role partners with the interdisciplinary team to ensure comprehensive resident assessments, accurate documentation, and appropriate reimbursement.

• Active Illinois RN license required

• MDS experience preferred

• Skilled nursing or long-term care experience required

• Strong assessment, organizational, and communication skills
