Alhambra Rehab & Healthcare is seeking an experienced and motivated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our leadership team as our MDS Coordinator / Infection Preventionist. This key leadership position combines clinical reimbursement expertise with infection prevention oversight to ensure exceptional resident care, regulatory compliance, and outstanding clinical outcomes.

Schedule Full-Time Monday–Friday8:00 AM – 4:30 PM Additional hours as needed based on facility needs

Participation in the management on-call rotation is required for weekend leadership coverage

CompensationRN: $39.00/hourLPN: $34.00/hour $10,000 Sign-On Bonus Sign-on bonus paid out over one year

ResponsibilitiesMDS Coordination Coordinate and manage the MDS assessment process in accordance with federal and state regulations.

Ensure accurate and timely completion and submission of all MDS assessments.

Lead interdisciplinary care plan meetings and collaborate with department leaders.

Monitor case mix, reimbursement opportunities, and quality measures.

Maintain survey readiness and support regulatory compliance initiatives.

Assist with clinical documentation and reimbursement optimization.

Infection Prevention Direct and oversee the facility's Infection Prevention and Control Program.

Conduct infection surveillance, tracking, investigations, and required reporting.

Identify infection trends and implement corrective action plans.

Educate staff on infection prevention policies and best practices.

Ensure compliance with CDC, CMS, state, and federal infection prevention regulations.

Promote a safe, healthy environment for residents, visitors, and staff.

Qualifications Current Illinois Registered Nurse (RN) or Licensed Practical Nurse (LPN) license required.

Previous MDS experience preferred.

Infection Preventionist experience preferred.

Strong knowledge of Medicare, Medicaid, reimbursement systems, and MDS processes.

Thorough understanding of long-term care regulations and infection prevention standards.

Excellent leadership, communication, organizational, and critical-thinking skills.

Ability to prioritize multiple responsibilities in a fast-paced healthcare environment.
