MDS Nurse / Infection Preventionist (RN)

This dual-role leadership position offers the opportunity to improve resident outcomes, support regulatory compliance, and positively impact the quality of care throughout our facility.

Schedule Full-Time Monday–Friday Participation in the facility's management on-call rotation is required

Compensation $39.00–$41.00 per hour, based on experience $10,000 Sign-On Bonus Sign-on bonus paid over one year

Position Overview This unique leadership role combines the responsibilities of both the MDS Nurse and Infection Preventionist, allowing you to play a vital role in resident care, reimbursement accuracy, regulatory compliance, and infection prevention. Your schedule will be divided between both areas of responsibility while collaborating closely with the interdisciplinary care team.

ResponsibilitiesMDS Coordination Complete and coordinate MDS assessments in accordance with state and federal regulations

Ensure accurate coding to support reimbursement and quality measures

Develop, update, and monitor individualized resident care plans

Collaborate with the interdisciplinary team to promote exceptional resident outcomes

Maintain accurate and timely clinical documentation

Infection Prevention Lead the facility's Infection Prevention and Control Program Conduct infection surveillance, tracking, investigations, and required reporting

Audit infection control practices throughout the facility

Educate staff on infection prevention standards and best practices

Monitor compliance during all shifts, including periodic off-shift observations

Ensure compliance with CDC, CMS, and Illinois Department of Public Health (IDPH) infection prevention guidelines

Qualifications

Current Illinois Registered Nurse (RN) license required

Previous MDS experience required

Infection Prevention experience preferred, but not required

Willingness to complete the CDC Infection Preventionist Training if not already certified

Strong assessment, organizational, communication, and leadership skills

Ability to work independently while collaborating effectively with an interdisciplinary team Participation in the facility's management on-call rotation is required
