Program(s): 830 - Supervised Residential: T he Supervised Residential Program  provides adults living with substantial levels of mental illness or disability with 24/7 services through recovery-oriented communityresidentialfacilities. Southern Illinois Rural Healthcare Services Outreach program (SIRHSO) - SIRHSOfocuses on expanding access to preventative health care services in underserved rural areas through the following four national priority areas:
Heart Disease, Cancer, Chronic Lower Respiratory Disease (CLRD), and Unintentional Injury. This consortium includes local healthcare providers and community-based partners who are committed to reducing health disparities and improving access to early detection and preventive services in our rural communities.

Immediate Supervisor:
Clinic Director
- Amber Fairless

Hours of Work:
Full-time, 40 hours/week. Schedule flexibility required to meet residential, clinic, and community needs.

Minimum Qualifications:

Active Illinois LPN license;
Graduate of an accredited nursing program.

LPN Skills & Knowledge:
Experience in behavioral health, residential services, community health, or integrated care settings; experience with electronic medical records and medication administration systems; experience providing chronic disease education and care coordination.

LPN Special Requirements:
Participation in after-hours medication consultation or on-call support as assigned; ability to travel to residential, clinic, and community-based locations; maintain and provide proof of a valid driver's license; must have reliable transportation; maintain and provide proof of personal vehicle liability insurance; at least 21 years of age; must complete required registry clearances and criminal fingerprint background checks; maintains CPR & First Aid training requirements.

Position Summary:

The LPN provides comprehensive nursing, medication management, and care coordination services for individuals receiving behavioral health, residential, outpatient, and community-based services. This position supports integrated physical and behavioral health outcomes through medication oversight, chronic disease monitoring, clinical consultation, health education, and coordination with internal teams and external providers. The RN plays a critical role in promoting continuity of care, reducing avoidable emergency department utilization, and ensuring compliance with nursing standards, agency policy, and applicable regulatory or grant requirements.

LPN job duties include, but are not limited to:

Coordinate medication management across assigned programs, including pharmacy pickup and delivery, medication reconciliation, verification of accuracy, audits, refills, and proper disposal of discontinued or expired medications.

Maintain accurate EMAR and paper MAR systems and ensure readiness for system downtime, emergencies, off-site visits, and provider appointments.
