This unique position combines MDS coordination and Infection Prevention, giving you the opportunity to make an impact across multiple areas of resident care.
You’ll work closely with nursing leadership and the interdisciplinary team to support accurate MDS assessments, care planning, reimbursement, quality measures, infection prevention, regulatory compliance, and positive resident outcomes.
MDS Responsibilities- Coordinate and complete MDS assessments
- Ensure accurate and timely documentation
- Support individualized resident care planning
- Monitor coding, reimbursement, and quality measures
- Collaborate with the interdisciplinary care team
- Coordinate the facility's Infection Prevention and Control Program
- Conduct infection surveillance, tracking, and required reporting
- Monitor infection control practices throughout the facility
- Educate and support staff on infection prevention practices
- Assist with regulatory compliance and survey readiness
- Complete periodic off-shift observations as needed
- Current Illinois Registered Nurse (RN) license required
- Previous MDS experience required
Infection Prevention experience preferred, but not required - Willingness to complete Infection Preventionist training if needed
- Long‑term care experience preferred
- Strong organizational and communication skills
- Excellent attention to detail
- Ability to work independently and as part of a leadership team
