Carrier Mills Nursing & Rehabilitation Center is seeking an experienced MDS Nurse Manager (RN or LPN) to join our leadership team.
This is a great opportunity for an MDS professional looking for a Monday–Friday schedule, competitive pay, a $10,000 sign-on bonus, and the opportunity to play an important role in resident outcomes and facility success.
Schedule- Full-Time
- Monday–Friday
- 8:00 AM – 4:30 PM
- Participation in the management on-call rotation is required
- Registered Nurse (RN): $33.00–$35.00/hour
- Licensed Practical Nurse (LPN): $29.00–$31.00/hour
- $10,000 Sign-On Bonus
- Sign-on bonus paid over one year
- Annual performance evaluation with the potential for a pay increase

- Coordinating and completing MDS assessments
- Leading and supporting interdisciplinary care planning
- Monitoring documentation, case mix, and reimbursement
- Supporting quality measures and survey readiness
- Maintaining compliance with CMS and regulatory requirements
- Collaborating with clinical leadership to improve resident outcomes
- Current Illinois RN or LPN license
- Previous MDS and care planning experience
- Long-term care experience preferred
- Knowledge of Medicare, Medicaid, and reimbursement systems preferred
- Strong organizational and communication skills
- Excellent attention to detail
- Ability to collaborate effectively with an interdisciplinary team
