• Provide direct nursing care to residents in accordance with physician orders, nursing standards, and established care plans.
• Perform ongoing resident assessments and document findings in the electronic health record (EHR).
• Administer medications and treatments safely and accurately according to established nursing standards.
• Monitor and communicate changes in resident condition to physicians and Registered Nurses.
• Implement and update individualized resident care plans in collaboration with the interdisciplinary care team.
• Provide resident and family education related to care, treatment plans, and facility guidelines.
• Supervise and direct Certified Nursing Assistants and support staff to ensure quality resident care.
• Perform IV therapy duties within the Illinois LPN scope of practice and facility policies.
• Ensure thorough shift to shift communication regarding resident status, physician orders, and pending care needs.
• Maintain compliance with infection prevention protocols, regulatory requirements, and hospital policies.
• Serve as a resident advocate while promoting resident rights, dignity, and well-being.
• Participate in department quality improvement initiatives and ongoing professional development activities.

$34.00 per hour + Shift Differential (Weekday Night: $2.00 per hour; Weekend: $3.00 per hour; Weekend Night: $5.00 per hour)

• Required:
  
  Current Licensed Practical Nurse (LPN) licensure in the State of Illinois;
  Current Basic Life Support (BLS) certification.
• Preferred:
  Long Term Care, Skilled Nursing, or Geriatric Care experience;
  Experience with electronic health records in a skilled nursing setting.

• Preferred:
  Previous nursing experience in a long-term care, skilled nursing, rehabilitation, or acute care setting;
  Experience supervising CNAs or other support personnel.
