• This role involves providing primary care services in a supportive, patient-centered environment.
• The position is full-time, located onsite in Silvis,IL, with a schedule of Monday through Friday, 9:00 am to 5:00 pm.
• The ideal candidate will have strong clinical skills and the ability to work autonomously while collaborating with an interdisciplinary team.

• NP or PA license in (insert whatever state it is in)
• Board Certification
• Ideally 1 Year of experience in an SNF setting, open to transferrable experience including new grads

• Must be Board Certified.
• Experience in primary care settings is preferred.
• Ability to assess patient health and perform physical examinations.
• Strong documentation and charting skills.
• Proficient in developing patient management plans.
• Experience in therapeutic procedures such as administering injections.
• Ability to provide patient counseling and wellness promotion.
