The Care Integration Senior Aide (CISA) conducts in-home visits to work directly with clients and Direct Support Professionals (DSPs). The CISA observes, documents, and reports changes in clients’ health and social conditions, collaborates with the care team to help prevent avoidable negative outcomes, and supports quality service delivery throughout the Community Supported Living (CSL) program.
Overall ResponsibilityProvide advanced, non‑clinical support to clients and Direct Support Professionals (DSPs) within the CSL program. The CISA focuses on observing, documenting, and communicating changes in clients’ health or social conditions and supporting quality service delivery through coaching and coordination.
While the CISA supports DSPs in the field, this role does not include direct supervisory authority and does not perform scheduling, disciplinary, or performance‑management functions.
Essential Functions In‑Home Visits- Coordinate with care managers to create, manage, and maintain schedules for in‑home visits.
- Conduct in‑home visits to observe and gather information regarding clients’ health and social conditions.
- Identify and help address worsening clinical or social conditions within the scope of the role and in accordance with agency policy.
- Support Direct Support Professionals in building trusting relationships and open communication with clients and families through coaching and guidance.
- Accurately document observations from in‑home visits and elevate concerns in a timely manner.
- Provide direct, hands‑on support to members of the CSL program as needed and within the scope of the position.
- Participate actively as a member of the interdisciplinary care team.
- Communicate observations from in‑home visits and receive direction regarding follow‑up activities.
- Share knowledge, perspective, and caregiver experience to support care planning and service delivery.
- Complete follow‑up activities delegated by care team members.
- Provide ongoing support to Direct Support Professionals and family caregivers.
- Maintain and update member assessment information to ensure records remain accurate and current.
- Train Direct Support Professionals, within the scope of the care plan, on implementation of identified supports.
- Provide updated guidance to direct support staff as client needs change or new information becomes available.
- Develop and maintain effective working relationships with co‑workers, clients, families, and community stakeholders.
- Comply with and promote agency policies, procedures, and values.
- Interact with others in a professional, respectful, and culturally responsive manner.
- Serve as a resource to Direct Support Professionals.
- Maintain knowledge of department‑specific issues, best practices, and available resources.
- Ability to participate in an on‑call rotation, as needed, to support unscheduled client and staff needs.
- Ability to recognize clinical and social concerns in the home and report critical information accurately and promptly.
- Strong commitment to caregiving and person‑centered support.
- Ability to remain objective, professional, and nonjudgmental when working with clients, families, and caregivers.
- Strong interpersonal, verbal, and written communication skills.
- Ability to use sound judgment, active listening, and problem‑solving skills.
- Ability to work effectively both independently and as part of a team.
- Strong organizational and time‑management skills, including the ability to manage multiple priorities in potentially stressful situations.
- Ability to follow instructions while exercising appropriate independent judgment.
- High school diploma or equivalent.
- One year of caregiver experience with the agency preferred, or two years of caregiver experience in a similar setting.
- Reliable transportation, a valid driver’s license, and proof of current automobile insurance.
