Then look no further! The Fairfax County Health Department (FCHD) is a progressive public health leader providing services that promote population health, protect public health and the environment, and ensure residents' equitable access to health services and information.

Under administrative supervision, performs advanced level professional duties and performs as the program unit supervisor for the Medical Care for Children Partnership (MCCP). Manages a multidisciplinary team to ensure the accurate and timely determination of MCCP eligibility and the coordination of supplemental health resources. A primary responsibility includes the generation and distribution of comprehensive monthly program reports to key stakeholders, including medical homes, pharmacy providers, the Health Department, and the MCCP Foundation.

Serves as a key member of the hiring panel, conducting candidate interviews and providing recommendations to ensure the selection of highly qualified staff. As a member of the management team, the Human Service Worker (HSW) IV identifies service delivery gaps, develops MCCP-specific standard operating procedures (SOPs), and maintains strategic partnerships to ensure program integrity and operational excellence.

• Thorough knowledge of complex federal, state and local laws governing public assistance and welfare reform;

• Knowledge of social, economic, health and cultural factors which can serve as barriers to employment;
• Knowledge of best practices in workforce development and case management;
• Ability to communicate effectively, both orally and in writing;

• Ability to read, interpret and apply program policies and procedures;
• Ability to use automated technology to conduct research, to establish and maintain case records, and to produce correspondence and reports;

• Ability to establish and maintain effective working relationships with clients, co-workers, other human service providers and the general public;

• Ability to develop, train, mentor, and motivate staff;
• Ability to review cases for consistency, best practice, timeliness, accuracy and appropriateness of action;

• Ability to recognize and implement best proactive approaches to service delivery;
• Ability to make public presentations.

Graduation from an accredited four-year college or university with a bachelor's degree plus four years of professional human services or related work experience.

• Valid driver's license.
• Cardiopulmonary Resuscitation (CPR) certification (Required within 30 days).
• Automated External Defibrillator AED certification (Required within 30 days).
• Basic Life Support (BLS) certification (Required within 30 days).
