The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. Position Summary

Seeking a Registered Nurse to work within the outpatient practice of the Center For Advanced Cardiac Care (CACC), Advanced Heart Failure, Pulmonary Arterial Hypertension, LVAD and Cardiac Transplantation program, under the leadership of the Chief Nurse Practitioner and direction of the CACC Medical Directors. The Clinical Coordinator III will be directly supervised by the Chief Nurse Practitioner. The RN will work collaboratively with Physicians, Nurse Practitioners, other members of multidisciplinary care team, and administrative support staff unto the CACC Program.

Responsibilities
  • Provide comprehensive patient care coordination and patient/caregiver education.
  • Support on-going clinical operations of the CACC advanced heart failure, LVAD and transplant program to provide compassionate, patient centered care.
  • Provide outpatient care including symptom follow-up and triage, coordination of diagnostic testing and outpatient clinic follow-up review of diagnostic reports, and supervised direct patient care.
  • Participate as an eager partner in the day-to-day management of patients and the vision of the CACC program.
  • Implement nursing care in the outpatient setting in collaboration with the Nurse Practitioners (NPs) and Medical Doctors (MDs) of the Center for Advanced Cardiac Care.
  • Work in the clinical-administrative office setting for patient care management.
  • Care for ambulatory patients with advanced heart failure, pulmonary arterial hypertension, LVAD and/ or pre- and post-heart transplant outpatients along the continuum of their disease including patients receiving medical therapies such as oral, intravenous, and inhaled medications.
  • Obtain and review laboratory and other diagnostic test results.
  • Evaluate symptomatic patient callers and conduct telephonic triage.
  • Provide initial and ongoing patient and caregiver education including but not limited to medication, diet, and lifestyle management.
  • Administer nursing care in the clinic as directed by their collaborating Physician and Nurse Practitioner partners.
  • Complete clinical documentation in the outpatient medical record.
  • Provide remote monitoring/assessment as prescribed by their Physician and Nurse Practitioner Collaborators.
  • Utilize excellent communication skills and remain flexible in a dynamic care setting for complex patient management.
  • Effective communication and relationships with all members of the continuous quality improvement team.
  • Demonstrate responsibility and accountability for own professional practice and seeks guidance in areas of question.
  • Conduct self in a professional manner in all internal and external relationships.
  • Take the initiative to make improvements and to resolve issues.
  • Can be relied upon to do what is best for the organization.
  • Maintain confidentiality of medical record and physician specific information.
  • Complete all mandatory education.
  • Maintain an appropriate level of awareness of and sensitivity to the legal and financial framework in which the institution operates.

  • Must demonstrate commitment to the highest level of quality.
  • Conform to all applicable HIPAA, billing compliance and safety guidelines.

Minimum Qualifications
  • Bachelor’s Degree in Nursing (BSN) or equivalent in education and experience, plus four years of nursing experience.
  • Current NYS RN license
  • BLS certification.
Preferred Qualifications
  • Telemetry, intensive care unit, coronary care unit, step-down unit, cardiology, cardiothoracic surgery, or emergency department experience preferred.
  • ACLS certification preferred.
Other Requirements
  • Basic level MS Office skills
  • Effective communication methods and skills; addresses issues in a positive and constructive manner.
  • Medical clearance and participation in the medical surveillance program
    • Contact with patients and/or human research subjects
    • Potential blood borne pathogen exposure
    • Wear N-95 respirator
  • Must successfully complete applicable compliance and systems training requirements.
