Compensation:
* Offering up to $7,500 sign-on bonus
* For LCSW, LPCC, LMFT:
Assuming Full-Time, 75 units/week (15 units/day), a Psychology provider could estimate they will earn $75k - $100k/year.
For Psychologist (PsyD/PhD) Assuming Full-Time, 75 units/week (15 units/day), a Psychologist (Ph.D. or PsyD.) provider could estimate they will earn $125k - $145k/year.

Job SummaryTelos Health Systems is seeking a Psychotherapist (LCSW, LPCC, LMFT, or Licensed Psychologist – PsyD/Ph.D.) to provide mental health evaluations and psychotherapy services to patients in Skilled Nursing Facilities and Assisted Living Facilities throughout the Bardwell, KY area.
This role offers flexibility, clinical autonomy, and strong administrative and leadership support.
- Conduct comprehensive mental health assessments and diagnostic evaluations.
- Provide individual psychotherapy and ongoing treatment services.
- Develop, implement, and update individualized treatment plans in collaboration with a multidisciplinary care team.
- Maintain accurate, timely, and compliant clinical documentation.
- Coordinate care with facility staff, medical providers, and other healthcare professionals.
- Adhere to all federal, state, and organizational standards, policies, and regulations.
Required
- Active Kentucky license as one of the following:
- Licensed Clinical Social Worker (LCSW)
- Licensed Professional Clinical Counselor (LPCC)
- Licensed Marriage and Family Therapist (LMFT)
- Licensed Psychologist - PsyD/Ph.D.
- Master’s degree (LCSW, LPCC, LMFT) or Doctoral degree (Psychologist) in a relevant mental health field.
- Ability to work independently while collaborating effectively within a clinical team.
- Strong communication and clinical documentation skills.
- Experience working with geriatric populations.
- Experience in Skilled Nursing Facilities, Assisted Living, or long-term care settings.
- Pay-per-encounter, fee-for-service compensation.
- Flexible scheduling with a focus on work-life balance.
- No billing or authorization responsibilities (handled by our billing team).
- Ongoing support from Directors of Mental Health and administrative staff.
