Key Responsibilities
- Conduct comprehensive evaluations to assess functional needs, develop individualized treatment plans, deliver evidence-based interventions, and monitor progress to maximize patient independence, safety, and quality of life.
- Deliver evidence-based occupational therapy in accordance with physician orders and the CMS Conditions of Participation (CoPs).
- Ensure timely, accurate documentation and maintain compliance with CMS, payer, and agency standards.
- Collaborate with patients, caregivers, physicians, and the interdisciplinary team to maximize function and independence.
- Educate patients and caregivers on ADLs, home safety, adaptive equipment, fall prevention, and individualized Home Exercise Programs (HEPs) to promote safety, independence, and functional recovery.
- Monitor patient progress, update treatment plans as needed, and communicate changes to the care team.
- Degree from an accredited Occupational Therapy program.
- Current Michigan Occupational Therapist license in good standing.
- Home health experience preferred (minimum one year).
- Strong clinical judgment, critical thinking, and problem‑solving skills.
- Excellent communication, interpersonal, and patient education skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
- Reliable transportation, a valid driver's license, and current auto insurance.
