Home Health Start of Care Registered Nurse (RN) 7 On 7 Off
Join our team at Specialized Home Care, a leading provider of home health care services, as a Start of Care Registered Nurse (RN) working a 7 on/7off schedule. As a member of our team, you will play a critical role in delivering exceptional patient care, establishing individualized plans of care, and promoting positive clinical outcomes.
Essential Responsibilities- Complete Start of Care (SOC) visits and comprehensive patient assessments in accordance with physician orders, agency policies, and regulatory requirements.
- Perform and accurately document OASIS assessments within required time frames.
- Develop individualized Plans of Care based on patient needs, physician orders, and clinical findings.
- Assess patients' physical, emotional, psychosocial, cognitive, environmental, and safety needs.
- Establish appropriate home health services and coordinate care with physicians, therapists, social workers, and other members of the interdisciplinary team.
- Full-Time
- 7 Days On / 7 Days Off
- Scheduled admission and Start of Care visits during assigned work week
- Weekend and holiday coverage required during scheduled rotation
- Active Registered Nurse (RN) license in the state of practice.
- Minimum of two (2) years of nursing experience.
- Current CPR certification.
- Valid's driver's license, reliable transportation, and current automobile insurance.
- Ability to travel throughout the assigned service area.
- Strong clinical assessment, critical thinking, and organizational skills.
