Join our compassionate and dedicated team as the Manager, Health Care Home. In this role, you will oversee the daily operations of the Healthcare Home (HCH) Team, coordinate HCH clients, facilitate health education groups and activities, and lead Nurse Care Managers in a culture of collaboration and excellence.
Key Responsibilities- Oversee and supervise the daily operation of the HCH Team.
- Coordinate and track HCH enrollees (new enrollees, discharges, transfers).
- Supervise Nurse Care Managers and assign NCM caseloads in partnership with the Assistant Director.
- Coordinate and facilitate health education groups and activities for clients, staff and community.
- Review, complete and submit the monthly Attestation report in Cyber Access promptly.
- Serve as Cyber Access Practice Administrator: manage user accounts and monitor agency usage and prescriber reports.
- Coordinate management of HIT tools and initiatives (Cyber Access usage, CMT reports, hospital admission/discharges).
- Oversee and/or facilitate staff training on Healthcare Home.
- Participate in monthly TA calls for HCH Directors and attend state‑required meetings and trainings.
- Oversee Quality Assurance and Quality Improvement for Healthcare Home.
- Partner effectively with collaborating service areas to meet client needs.
- Current Registered Nurse (RN) in the state of practice.
- Supervisory experience.
- Strong verbal and written communication skills.
- Computer and organizational skills.
- Ability to complete a criminal record and child abuse & neglect background check prior to employment.
- Complete New Hire Orientation within 30 days of employment.
- Complete all required training (e.g., Relias Learning) within two weeks of employment and annually thereafter.
