Registered Nurse Clinical Manager

Center Well Home Health, part of Humana Inc., seeks a Registered Nurse Clinical Manager to lead our in-home nursing team serving seniors and adults with chronic conditions. In this role, you'll oversee patient assessments, care planning, and clinical quality while mentoring field nurses and ensuring regulatory compliance. Responsibilities

• Lead and supervise a team of home health nurses delivering in-home clinical care
• Oversee patient assessments, care planning, and care coordination for seniors and adults with chronic conditions
• Ensure compliance with clinical protocols, regulatory standards, and documentation requirements
• Collaborate with an interdisciplinary team to optimize patient outcomes and independence at home
• Provide clinical guidance, mentorship, and performance feedback to nursing staff
• Monitor quality metrics and implement improvements in patient safety and care effectiveness
• Engage with patients and families to support education, care plans, and patient-centered goals
• Coordinate scheduling and resource allocation to meet patient caseload demands in the field
