At Phoenix Hospice, our Admissions RNs are the first calm presence when hospice care begins. You'll guide patients and families from curative care into comfort-focused care with compassion, clinical excellence, and dignity. This role blends strong clinical judgment with deep human connection—addressing not only physical needs, but also emotional, spiritual, and psychosocial support.
You'll collaborate closely with intake coordinators, hospice consultants, medical directors, facility staff, and physicians to ensure every admission is smooth, thoughtful, and centered on what matters most.
Requirements- Hospice experience required
- Active Missouri or Compact RN license
- Valid driver's license
- Strong assessment, organization, and communication skills
- Ability to work independently and as part of a team
- Complete hospice admissions and skilled nursing visits
- Provide compassionate care and support to patients and families
- Educate patients and families on conditions such as CHF, COPD, and diabetes
- Provide wound care and medication management
- Document patient conditions and communicate changes to physicians
- Collaborate with the interdisciplinary hospice team
