We are seeking compassionate Registered Nurses (RNs) to join our team as Case Managers, working one-on-one with patients in their home environment. As a Case Manager, you will establish and oversee all aspects of patient care, creating a Plan of Care and following physician orders to achieve outcomes of increased patient independence and satisfaction. • Assume responsibility for assigned patient's care from admission through discharge, effectively executing patient's Plan of Care
• Collaborate with patients, family/caregiver(s), ordering physicians, other care team members, and community resources
• Submit accurate, complete, timely, meaningful, and required documentation, including progress and clinical notes
• Participate in case conferences with other clinicians, reviewing services, clinical documentation, and discussing pertinent patient care issues and goals
• Participate in on-call rotation as reasonably assigned

• Minimum of 2 years of nursing experience, with a preference for 2+ years in an acute or post-acute care setting
• Active State Registered Nurse licensure required
• Proof of current CPR certification and valid Driver's license
