LPN​/Visit

Full Time Licensed Practical Nurse (LPN) in Independence, Missouri, providing direct patient care under RN supervision in a supportive environment. The role involves medication administration, documentation, and collaboration with interdisciplinary teams to ensure quality patient outcomes.

• Provide direct patient care under supervision of registered nurses and providers

• Administer medications and treatments per physician orders and protocols

• Monitor patient conditions and report changes to the clinical team

• Accurately document patient information, treatments, and care plans in Matrix EMR

• Assist with admissions, discharges, and ongoing assessments

• Collaborate with interdisciplinary teams to ensure quality outcomes

• Educate patients and families on treatment plans and care instructions

• Maintain compliance with state, federal, and facility regulations

• Deliver compassionate, patient-centered care in a fast-paced setting

• Graduate of accredited LPN program

• Active Missouri LPN license

• Compact nursing license or active Missouri and Kansas LPN license

• Wound care and catheter care experience

• Strong communication, organizational, and clinical assessment skills

• Ability to work independently and collaboratively within a healthcare team

• Minimum 1 year LPN experience

• Current CPR/BLS certification

• Experience with Matrix EMR

You hold an active Missouri LPN license. You have wound and catheter care experience. You are proficient with Matrix EMR and BLS certified
