Carmel Hills Wellness & Rehabilitation is seeking a compassionate and driven Social Services Designee to support the emotional, social, and psychological well‑being of our residents and their families. If you’re passionate about advocacy, dignity, and person‑centered care in a skilled nursing environment, we’d love to connect with you.

• Conduct psychosocial assessments and contribute to individualized care plans

• Support residents and families through transitions into long‑term care and memory care

• Complete MDS‑related documentation accurately and in compliance with regulations

• Advocate for resident rights, dignity, and self‑determination

• Coordinate discharge planning and connect residents with community resources

• Collaborate with nursing, administration, and the interdisciplinary team

• Assist with survey readiness and regulatory compliance efforts

• Certified, licensed, or registered in the state of practice (required)

• Bachelor’s degree in Social Work or a related Human Services field

• Minimum of 1 year of social work experience in a healthcare or geriatric setting

• Skilled Nursing Facility (SNF) experience preferred

• Nursing experience in a SNF is a plus
