JOB TITLE:

RN Case Manager / Nursing Supervisor

  • This hybrid role requires a mix of case management, recertifications, starts of care (SOC), resumptions of care (ROC), other assessments and services, occasional Skilled Nursing visits, as well as new hire orientations, trainings, in-services, and supervisory reviews.
  • At least 1 year of Case Management experience and home health agency (or equivalent) experience are required
    .
    • Must have experience with OASIS and NON-OASIS assessments.
    • 1 or more years of experience in home health care, wound care, lab draws, and OASIS assessments is a plus.
SERVICE AREA: 4+ county radius in North East Ohio
  • Includes Summit, Medina, Cuyahoga, Tuscarawas, and surrounding Counties, primarily aligned along I-77
  • Typical travel distances require a 30‑90 minute daily radius (or less), with occasional travel up to 2 hours to cover all county cases also required.
JOB LOCATION OPTIONS:
  • Qualified applicants may reside in or near any of the primary service area counties so long as travel requirements are fulfilled to all stated counties.
  • Primary Service Area Counties include:
    Summit, Medina, Cuyahoga, Tuscarawas, primarily aligned along I‑77.
  • Additional Counties where RNs may reside to serve the counties above include:
    Stark, Wayne, and Holmes.
SCHEDULE TYPE:
  • Flexible and customizable. We'll build a schedule as close to home as possible - that fits your availability and preferences.
  • Typical hours are between Monday - Friday (weekdays) during the day (8am - 5pm).
EMPLOYMENT TYPE:

Part‑time + Long‑term employment*
  • No On‑Call requirements / No Overnight shifts required.
  • HCN will expand this role into Full‑Time (choice of hourly or salary) for the right candidate.
  • Weekend, Overtime, Respite & PRN services are offered when available but are not required.
  • Part‑Time RNCMs receive hourly compensation (paid weekly, on Fridays).

  • This position requires travel to multiple counties but offers a consistent schedule to fit your personal / family commitments (no overnight shifts or overnight travel).
  • This is a Part‑time, long‑term employment opportunity that can be built to fit your schedule (with potential to expand into permanent Full‑time position after 30‑90 days).
