Overview
JOB TITLE: RN Case Manager / Nursing Supervisor
This hybrid role requires a mix of case management, recertifications, starts of care (SOC), resumptions of care (ROC), other assessments and services, occasional Skilled Nursing visits, as well as new hire orientations, trainings, in-services, and supervisory reviews.
Responsibilities- Coordinate and provide patient care, oversee patient services, and ensure optimal outcomes with core skills in managed care, patient assessment, treatment plan documentation, discharge planning, and communication among physicians, agency staff, service administrators, other caregivers, clinicians, and the client/family.
- RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
- RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
- Document/Communicate/Coordinate patient status or changes in condition to the care team.
- Promote personal safety and a safe environment for clients and coworkers; travel to surrounding counties as required.
- Willingness to travel across stated counties; flexibility to align schedule with personal/family commitments.
- Graduate of an accredited School of Nursing; active RN License to practice in the State of Ohio.
- Minimum of 1 year experience as a Registered Nurse; 1 year experience in case management and home health required; 2+ years of home health experience highly preferred.
- CPR and First Aid Certifications required (or obtainable prior to hire via agency or accredited third parties).
- Experience in supervisory roles; experience with wound care, lab draws, and OASIS assessments is a plus.
- Willingness to travel throughout all stated counties in the service area; familiarity with HIPAA regulations and medical terminology.
- Current Driver's License and reliable transportation; strong confidentiality and dependable attendance.
- Desirable traits: reliable, professional, compassionate, detail-oriented (care and documentation), accountable, respectful, and positive.
- Service area: 4+ county radius in Northeast Ohio, including Summit, Medina, Cuyahoga, Tuscarawas, and surrounding counties, primarily along I-77.
- Typical travel radius: 30–90 minutes daily, with occasional travel up to 2 hours to cover all county cases.
- Qualified applicants may reside in or near any primary service area counties; travel requirements to all stated counties must be fulfilled.
- Primary Service Area Counties include Summit, Medina, Cuyahoga, Tuscarawas (primarily along I-77); additional counties:
Stark, Wayne, and Holmes. - Flexible and customizable schedule
. Typical hours Monday–Friday, 8am–5pm; no on-call requirements or overnight shifts. Role may expand to Full-Time (hourly or salary) for the right candidate. Weekend, overtime, respite & PRN services offered when available but not required. Part-Time RNCMs compensated hourly.
- Part-time hourly wage: $35 - $40/hour
- Part-time RNCMs eligible for quarterly bonuses; full-time RNCMs eligible for quarterly bonuses, PTO, health coverage, and other benefits.
