Suncrest Hospice - - Responsibilities:
Provide resident care to assigned residents under the direction of a Case Manager or Director of Nursing;
Coordinate with licensed care staff to deliver appropriate care per care plans;
Follow nursing policies, procedures, and safety guidelines;
Work as part of a patient care team and communicate with nurses, social workers, chaplains, volunteers, and leadership;
Provide 1-1 patient interaction and support while maintaining independence as needed
